 2012 Quilt Show

You can download each of the following forms by clicking on its title/link.  Clicking on the link in the normal way will open the file itself and then you save it from within the application.  Alternatively, making a right/context-sensitive click will allow you to choose "save target as" from the context-sensitive menu and download the file without having to open it first.

Quilt Registration form (.doc) (11 Jan 2012: new version of form uploaded)

Quilt Registration form (.pdf) (hardcopy alternative, see note below) (11 Jan 2012: link currently disabled; will be re-enabled when the new version in .pdf is available)

Note:  The Quilt Registration Committee asks that you fill out the .doc quilt registration form and submit the completed form via email.  It will save them much time and effort compared to if you print the .pdf form and submit it filled out in hardcopy.  However, if you cannot fill in your quilt registration information on the .doc form, then the .pdf version submitted in hard copy is an option for getting your quilt registered.  The committee also requested that you submit your quilt registrations as soon as possible.

 

The Peddler's Table and Silent Auction Committees would also appreciate their forms filled out electronically and submitted via email.  .pdf versions are not available right now.
